Admission Checklist

  • The non-refundable application fee of RM300 and testing fee RM200 (RM500 total--checks payable to Dalat International School Sdn Bhd).
  • Principal/Counselor Reference form.
  • Current picture of the applicant.
  • Photocopy of birth certificate.
  • Previous 2 years school records, including latest grades available and standardized test scores if available (not applicable for Preschool and Kindergarten applicants).
  • Student Health form, including immunization records.
  • Photocopy of student passport.

  • Physical Evaluation form.
  • Photocopy of father's and mother's passport.
  

Application Forms

For students applying to preschool and kindergarten, complete all the forms in the P-K Application Package (PDF). This package includes:

  • Student Application
  • Health Form
  • Physical Evaluation

For students in grades 1-12, complete all the forms in the Grade 1-12 Application Package (PDF). This package includes:

  • Student Application
  • School Records Request Letter
  • Teacher Reference Form
  • Principal/Counselor Reference Form
  • Student Health Form
  • Physical Evaluation

For students who have previously attended and are returning, please complete the one-page Application for Returnees

For students who will become resident boarders, please complete the Boarding Application Package.
