All forms should be submitted electronically, faxed or mailed to the Director of Dalat International School. We have provided two options in accessing the forms required:

Download the PDF form to your computer, print it, fill it out and then mail it to us, or

Fillout the PDF forms online and email them to us.

Keep a copy of all records and forms you complete and send to the school. Be sure to sign and date all forms. If filing out hard copies, please type or print legibly in ink. Carefully follow all directions for completing any paperwork. All forms are also available from the Dalat Download Center or click on links below.
